These Terms of Service ("Terms") govern your use of the website www.onecarecdl.com and the services provided by OneCare CDL ("we," "us," or "our"), operated by Dr. David Jacobsen in Daytona Beach, Florida. By using our website or services, you agree to these Terms.

Services

OneCare CDL provides the following occupational health services:

  • DOT/CDL physical examinations (FMCSA-certified)
  • Non-CDL DOT physical examinations
  • DOT drug testing (instant and lab-confirmed)
  • DOT breath alcohol testing
  • Pre-employment physical examinations

All examinations are performed by Dr. David Jacobsen, a certified medical examiner listed on the FMCSA National Registry.

Appointments & Scheduling

  • Appointments can be booked online through our Square scheduling system, by phone, or as walk-ins during business hours.
  • We appreciate advance notice if you need to cancel or reschedule. Missed appointments without notice may affect future scheduling.
  • Walk-in availability is subject to current patient volume. We recommend booking in advance when possible.

Payment

  • Payment is due at the time of service unless prior arrangements have been made with your employer or fleet company.
  • We accept cash, credit cards, and debit cards. Card payments are processed securely through Square.
  • Current pricing is listed on our website. Prices are subject to change; the price at the time of your appointment applies.
  • We do not bill insurance for DOT physical examinations or drug tests, as these are typically employer or driver-paid services.

Medical Examinations

  • DOT physical examinations are conducted in accordance with FMCSA regulations (49 CFR Part 391). The medical examiner's determination is based on federal guidelines and clinical judgment.
  • Examination results are reported to the FMCSA National Registry as required by federal law. This is not optional and cannot be waived.
  • You are responsible for providing accurate and complete medical history. Providing false or misleading information may result in an inaccurate determination and may violate federal regulations.
  • If you disagree with an examination result, you may seek a second opinion from another FMCSA-certified medical examiner.

Drug & Alcohol Testing

  • Drug and alcohol testing is conducted in accordance with DOT regulations (49 CFR Part 40).
  • Test results are reported to the designated employer representative (DER) or consortium as required by regulation.
  • Refusal to complete a test, tampering with a specimen, or failure to appear for a scheduled test may be treated as a positive result under DOT regulations.
